Qualifications

• Active New York State RN or LPN License

• Current CPR Certification

• Strong communication and patient care skills

• Ability to provide compassionate, individualized patient care

• Ability to work independently and collaborate with healthcare professionals

• Home care experience preferred but not required

About the Role

As a home care nurse with Serene, you will provide skilled, individualized nursing care to patients in their homes throughout Suffolk County.

Responsibilities may include:

• Providing direct nursing care based on individualized patient care plans

• Monitoring patient conditions and documenting care

• Administering medications and treatments as prescribed and within your scope of practice

• Communicating with families, physicians, and clinical supervisors

• Supporting patients with their daily healthcare needs

• Maintaining a safe, comfortable, and supportive care environment

Pay Ranges

Registered Nurse (RN): $45–$48/hour

Licensed Practical Nurse (LPN): $35–$38/hour
